USA (West St. Paul, Minnesota) – Targeted Case Management Service

(1) Vendor needs to provide targeted case management service to the government authority located in West St. Paul, MN.                                            - To adults with serious and persistent mental illness(es) (SPMI) and co-occurring substance use disorders.- These services assist individuals with identifying, gaining, and coordinating access to needed behavioral health care, benefits and entitlements, social/housing/vocational services, and other needed supports to help individuals thrive in the community.(2) All questions must be submitted no later than June 22, 2026.
