USA (Utah) – Laboratory Services

(1) Vendor needs to provide laboratory services to the government authority located in Utah.• Receive, log-in, and track samples as they are delivered.• Provide agency within two working days of sample receipt a copy of the completed chain-of-custody record that accompanied the samples during shipment a completed condition of samples upon laboratory receipt form, and a register form identifying the analyses that are scheduled for the samples.• Maintain and document chain-of-custody for each sample as it progresses through the laboratory, including disposal. Chain-of-custody forms will accompany each sample shipment documenting custody from field personnel to laboratory personnel.(2) All question must be submitted no later than June 26, 2026.(3) A contract period will be for five years.
