Scope of Work
(1) Vendor needs to provide mobile medical services to the government authority located in Seattle, WA.• Mobile medical services which may also include behavioral health and social services• Provide services that assess and address the medical, behavioral health and social service needs of low-income older adults (aged 55+)• Services should include a mix of outreach, medical services, behavioral health services and social services• Services delivered in this model will be nurse-led and informed by stakeholder engagement regarding local needs.• Providers will establish trust and rapport with seniors, using engagement, treatment and patient-centered care best practices.• Services will focus on screening/assessment, provision of basic medical care, and linkages/referrals for other care needs• Registered Nurse to assess and respond to medical and behavioral health conditions; to conduct triage, provide patient wound and foot care, health education, and coordinate client care; facilitate linkages/referrals and ongoing connection to care, including relevant urgent, primary, behavioral health• Medical Assistant to assist in patient care, blood pressure checks, diabetes screenings, and drawing labs, in partnership with the agency role will also include engaging clients, building trust, and linking clients to ongoing services.(2) The contract period will be for one year.(3) All the questions must be submitted no later than July 7, 2026(4) Pre-bid meeting will be held on June 29, 2026
