USA (Brick, New Jersey) – Physical Therapy Services

(1) Vendor needs to provide physical therapy services to the government authority located in Brick, NJ. • Responsible for physical evaluations of students assigned to him/her.• Conduct therapy sessions in accordance with IEP and other medical professionals’ instructions.• Contribute to the development of Individual Educational Programs.• Responsible for re-evaluation of classified pupils as outlined in the state rules and regulations.• Participate in meetings concerning individual students when requested.• Consult with parent(s), students, members of the Child Study Team, and other staff personnel.• Work cooperatively with community agencies, in concert with the District, concerned with thephysical health of children.• Facilitate pupil growth and progress toward stated objectives of Physical Therapy• Maintain and submit reports and records as needed by the Supervisor of Pupil Personnel Services within the timeframe required by District staff(2) The contract period will be for one year.
