USA (Delaware) – Advanced Planning Services for Individuals

(1) Vendor needs to provide advanced planning services for individuals to the government authority located in Delaware.- Provide advance planning and other legal decision-making planning for agency recipients- Develop advance planning directives and other legal decision-making documents that will support choice and autonomy in decision-making, as well as promote access to care.- Community inclusion and participation in activities that are important to service recipients- Employment (applicable core indicator: the proportion of people who express they want a job who have a related goal in their service)(2) A non-mandatory pre-bid meeting will be held on July 21, 2026.(3) All questions must be submitted no later than July 7, 2026.(4) The contract period will be for one year.
